Default 4000ppi, then interpolate in software that does the job as
harmlessly as possible. Scanning at other than the optical res will
usually just mean interpolation in the scanner driver by relatively crude
means, like nearest neighhbour.
I'd strongly suggest that you use QImage
http://www.ddisoftware.com/qimage/ for this, since - although it has a
weird UI, it works brilliantly. Interpolation to the correct res and
appropriate sharpening are handled automatically in the background, and
all you have to do is drop the file onto a template of the page.

Resample (bicubic) is the best method within PS. If you want better, try
incremental as you suggest, or use one of several PS plugins.
But QImage will do it better and easier. Or, use Lanczos interpolation in
Irfanview.

QI means you only have the one, 4,000ppi file. Otherwise, save both, if you
think you'll need 'em.
