Sorry for the slow response.
I'd hazard a guess that in 3 or so years of disuse, either lubricant on
the carriage mechanism has dried out so the scanner motors are unable to
rack the carriage through initialisation, or you have a poor plug connection.
I will be putting up a page about DIY servicing of the '4000 ASAP which
may help. I didn't plan to, but mine just threw a wobbly right when I was
on deadline to scan some 1983 negs, and I *had* to fix it, and this time
took a few photos whilst it was apart.
